Full job description

Clinical and Care Professional Leads support south east London-wide and borough programmes of work, therefore, there is an expectation that you currently work within south east London, and for borough based roles, within that borough.

  • Challenge assumptions
  • Influence planning and delivery
  • Provide clinical and professional knowledge
  • Bring skills from their front-line roles and organisations
  • Lead and contribute to meetings and programmes of work
  • Consider local needs alongside evidence-based practice
  • Contribute to debate and development
  • Apply quality improvement practice and populational health management

As a CCPL lead you will have responsibility for specific portfolio and subject matter areas within one of four strategic portfolios. We invite applications for the following strategic portfolios:

  • Adult portfolio including Planned Care, Urgent and Emergency Care (UEC), Cancer, Prevention (Vital 5), Sexual Health, Community Mental Health (CMH) Adults, Learning Disability and Autism (LD&A), Neighbourhood, Medicines Optimisation, and Long Term Conditions (LTC). 4 sessions per week available.

The South East London Integrated Care System (SELICS) brings together the organisations responsible for health and care services in south east London, making the greatest possible contribution to the health and wellbeing of people living in our six boroughs.

SELICS is made up of staff working in many different roles, in different settings, employed in different ways, by a wide range of partner organisations, and are at the centre of our collective ambition for greater integration and better care. Employing organisations include NHS Trusts, GP practices, Social Care and Voluntary sector organisations, social enterprises, dental surgeries, pharmacies and optometrists. We have an ambitious People Strategy that brings these partners together to deliver the ‘one workforce’ ambition, making the health and care system a better place to work and live.

  • Provide leadership and support to the areas of work aligned to your portfolio including attending your portfolio’s leadership meeting, as well as linking into south east London Integrated Care System transformation programmes (where relevant)
  • Develop and strengthen partnerships by building relationships with key partners to help deliver our vision of being more inclusive in our approach to planning, redesign, and delivery
  • Connect and build trust with colleagues and residents across traditional boundaries – developing strong networks and relationships that work in service of residents over organisations, places, or professional groups
  • Ensure that service delivery is person centred, outcomes focussed and protective of individual resident’s dignity
  • Play an ambassadorial role for Partnership Southwark, promoting a culture of openness, transparency, equality and diversity
  • Being a member of our Clinical and Professional Advisory Group which will meet quarterly and provide advice and guidance to the Partnership Southwark Strategic Board.
