Assess clients needs and strengths through a collaborative approach, ensuring that all clients have care plans and that these are implemented and regularly reviewed/updated. Communicate effectively with relevant agencies to ensure that an integrated programme of care is provided throughout. Develop and maintain effective working relationships with inpatient facilities regarding admission and discharge of clients. Participate in partnership working with other care coordinators in the admission, discharge and transfer of clients in accordance with Trust Policies.
Operate as a resource of specialist knowledge and expertise for community support providers (Supporting People providers, Housing Associations, Landlords) with the aim of reducing re-admissions and clients moving back to higher levels of support. Facilitate assessment by potential move-on accommodation providers at an appropriate time. Liaise with all parties and support clients practically and emotionally through the assessment and move-on process, including visits, transitional leave and final move. Provide clients and carers with verbal and written up to date information on the services offered by the Trust, how to express their views, complaints procedure.
Raise safeguarding concerns for clients in placements when necessary, monitoring the progress of any Enquiry and assisting with investigation and implementation of any subsequent Safety Plans. Monitor the therapeutic effects and side-effects of prescribed medication providing the client (and others as required) with advice and information. Develop and maintain a high standard of practice, based on assessment, using a range of psychosocial approaches, within a health and psycho-social framework. Refer carers for their own needs assessment to monitor efficacy.
Liaise with both the Local Authority and SLP / ICB for jointly funded placements. Attend the Accommodation Panel as needed.
