Full job description

An exciting opportunity has arisen for an admissions and discharge coordinator to cover a period of maternity leave. You will work with the multidisciplinary team liaising with patients, families and other agencies within health and social care to plan and facilitate timely admissions and discharges from the rehabilitation ward.

The ward has 14 intermediate care patients serving Lambeth and Southwark, and 6 beds for patients with neurological illness or injury. The ward offers specialist rehabilitation to enable patients to recover and regain mobility and independence following illness or injury.

The admissions and discharge coordinator supports the service to deliver high standards of patient care through robust pathway management, facilitating timely transfers of care from hospital or home onto the rehabilitation ward, and discharge to home/external accommodation providers.

You should have experience of working in a health or social care setting with adults and older people and knowledge of the field of patient rehabilitation, admission and discharge planning. Excellent written and verbal communication skills and an ability to organise and prioritise your workload are essential. You will demonstrate flexibility and the ability to support change in a fast-paced environment.

The post holder will be responsible for facilitating the improvement of the quality, effectiveness and efficiency of patient admission and discharge planning processes on the Pulross Rehabilitation Ward. The post holder will be required to act as a flexible administrative resource supporting the patient admission and discharge planning processes from the unit. The post holder will be required to lead on admission and discharge planning process improvement projects thus enabling patients to achieve a safe, effective and timely discharge from the unit. The post holder will also be responsible for triaging referrals made to the Intermediate care beds.

The post holder will report to the Deputy Clinical Lead for Pulross Intermediate Care Therapy with input also given from the Ward manager and Clinical Manager from the 2b neuro rehabilitation service.

You will form an integral part of the multi-disciplinary team (MDT) on the Pulross Rehab Ward, including Consultants, Doctors, Physiotherapists, Occupational Therapists, Speech and Language Therapists, nursing staff and admin staff.

You will also work closely with colleagues from Lambeth and Southwark Social Care and other external agencies.
