Full job description

Clinical and Care Professional Leads support south east London-wide and borough programmes of work, therefore, there is an expectation that you currently work within south east London, and for borough based roles, within that borough.

The primary responsibility of the STAY WELL CCPL will be to provide strategic leadership, operational insight and clinical input to support meaningful integration of primary, community and secondary care services, to meet the ever-growing need within our Greenwich population.
The role also holds responsibility for WORKFORCE, a critical enabler to our health & wellbeing plan.

Interview date: 6 or 7 th June – exact time/date tbc

  • Facilitating the development of a clinical vision for community-based care in Greenwich, making the Fuller vision a reality in Greenwich, through better joining up care and improve population health outcomes, across the borough, as well as through neighbourhoods.
  • Playing a key role with our emerging primary care collaborative, building future approaches in conjunction with our Primary Care Networks on model development, to build encourage the leadership and sustainability of primary care model of the future
  • Champion the appropriate use of Population Health systems to deliver better outcomes and development of enhanced service models at the appropriate scale for the needs identified
  • Being the Stay Well representative within the Healthier Greenwich Partnership and, ensuring there is alignment and appropriate resource allocation to deliver on the Health & Wellbeing Strategy and annual plans

Employing organisations include NHS Trusts, GP practices, Social Care and Voluntary sector organisations, social enterprises, dental surgeries, pharmacies and optometrists.

  • Representing Greenwich at a South East London level as well as on a number of key strategic meetings such as the Health and Wellbeing Board, Neighbourhood and Health Inclusion Steering Group, and the Healthier Greenwich Partnership
  • Workforce is a key enabler – To provide clinical leadership on workforce development across the partnership, with a particular focus on primary and community care. This will include championing recruitment, retention, and promotion of NHS careers through the health ambassador programme
  • Oversee the CCPLs for Long Term Conditions and Cancer, and Primary and Secondary Care interface – ensuring join up and collaboration is effective across the portfolios
