Lewisham and Greenwich NHS Trust is an integrated community and acute trust providing a seamless service to residents of the borough

We currently have opportunities for Community Band 6 nurses to join our team, working in the London Borough of Lewisham. We cover all areas of the Lewisham Borough and the role could be based in any one of our 4 neighbourhoods.

You might already be a band 6 looking for your next role, or an experienced band 5 nurse with hospital or community experience looking to develop your skills and knowledge. Either way we can support you with a robust training package which will give you the skills you need to succeed.

The successful candidate will be experienced in caring for patients with a variety of long term conditions and, with a desire to develop their leadership skills. Applicants will have the opportunity to apply their knowledge and evidence-based practice to community care.

The aim of our service is to enable patients to stay at home longer, and leave hospitals faster, maximising their potential for independence and wellbeing.

As no two days are the same in the community, you will be working with a large degree of autonomy and accountability, with the care you are giving makes a real difference to your patients and those important to them.

Our community service operates from 08:00 – 20:00, with the usual shift patterns being 08:00 – 16:00 and 12:00 – 20:00. The post involves some weekend working on a rota basis.

You will have a key role in the provision of high-quality care to clients in their own home, in addition to developing leadership and management skills to support members of your team. To succeed in our aim for patients to stay at home longer, our community teams work closely with members of the wider multi-disciplinary teams to plan and deliver complex care
