An exciting opportunity has arisen to be part of a unique national oncology service, hosted by Chelsea and Westminster Hospitals.
The UK Cancer and Transition Service (UCATS) provides integrated care for gender diverse patients who have experienced cancer. UCATS is operated by the Department of Oncology at Chelsea and Westminster Hospital and hosted within the TransPlus Gender Identity Pilot at 56 Dean Street.
In our second year of operation, w e are looking for a n enthusiastic, passionate and motivate clinical nurse specialist to join our multidisciplinary team. This role will involve direct s upport to patients, as well as clinician educatio n and promotion of the service. In collaboration with the administrator and the rest other team members, you will also will help develop the service through patient and public involvement activities.
As a team player they will need to be able to demonstrate excellent listening and communication skills, be organised, adaptable and flexible, show empathy and concern for others.

Main duties of the job
The post holder will contribute to the delivery of high quality, safe and compassionate healthcare to patients by providing an administrative service focused on the patient and their individual needs.
The patient will work together with the UCATS team of doctors and a specialist nurse to respond to :
- Provide timely response to referrals and gather further clinical information from pateints and their care teams
- Respond to clinical enquiries and escalate to other members of the UCATS team when needed
- Attend UCATS virtual clinics (1 st and 3 rd Mondays of the month)
- Provide direct support to patients via the telephone pre- and post-appointment .
- Support development of our clinical registry
- Assist with development of and collection of experience and outcome measures by supporting patient and public involvement meetings , and developing written and electronic questionnaires.
- Promote the service through local, national and international presentations

Roles and responsibilities
Support the delivery of a high quality healthcare service ensuring that all patients receive a high standard of service delivery
Handle queries from patients/relatives confidentially and sensitively; refer advanced clinical related queries to appropriate staff
Interact with patients and relatives in distress in a sensitive and empathetic manner
Provide an excellent standard of service to patients at all times
Ensure prompt response to clinical queries from patients and healthcare teams
Assist with development written and electronic questionnaires.
Liaise with patients and clinical teams to gather appropriate clinical information at referral and during follow-up
Clinic bookings and notification of patients and their clinicians
F ollow-up on referrals
Provide direct patient support v ia telephone
Attend UCATS virtual clinics
C o-develop patient outcome and experience measures
Assist in developing the clinical registry
Assist in developing guidelines and educational materials
Promote the service through local, national and international presentations to clinicians, patient groups and other stakeholders.
Assist the administrator with responsibility for the day to day management of the service
Set personal and professional objectives
Seek education in the field and keep up-to-date with relevant services to signpost patients to
Actively participate in audit, research and the evaluation of the role and the service
Promote good practice and adopt a holistic approach to patient care at all times
