Full job description
We are looking for an enthusiastic mental health practitioner to join our small friendly, nurse led team.
You will require experience of working in a mental health setting and awareness of delivery of community mental health care.
You will hold and manage a caseload of people (for up to 6 months) facilitating assessment, care planning, sign posting, short term interventions and safe discharge. Your role will also include liaison with other multi agency services.
The team covers an ethnically and culturally diverse area, and you will have the opportunity to work with clients from a wide range of backgrounds, and with a wide range of psychosocial needs. By assessing your clients and their carers needs you will be able to access and sign post to a wide range of community resources and provider services available in the local area.
As we recognise the importance of supporting our staff to achieve an effective work-life balance and also maintain positive wellbeing, we work with staff to develop ways of supporting flexible working for them.
Assessments, care planning, psychosocial interventions, safeguarding and care connecting.
We work closely with probation, approved premises and Greater Manchester Police as part of safe risk management.
At times we work in a liaison role with our partner agencies – supporting vulnerabilities and mental health concerns.
