This is an opportunity to become part of a team that provides nursing care to children in a variety of community settings.
The Continuing Care Team provides clinical nursing care, training and support to children, young people (aged 0-18) and their carer’s at home or other community settings, enabling them to remain safely in the community and transition to adult services where appropriate.
Our team look after the following children and young people in the community:
Children with acute and short-term conditions
Children with long term conditions
Children with disabilities and complex conditions, including neonates
Children with life-limiting and life-threatening illness, including palliative and end-of-life-care.

To work as part of an integrated team with other professionals, providing hands on care and attention to children/young people with a range of complex health
To undertake a wide and varied range of trained clinical, administrative and supportive tasks as directed by the Clinical Team Lead and supervisor.
To effectively provide input in the assessment of children/young people’s health and social care needs and to implement the plan of care and evaluate individual
To work in partnership with other agencies to ensure children/young people and their families are offered support and assistance as required in the clinical setting in their home as required
To provide high standard of care to children at home with support from the clinical lead.
To assume responsibility for delegated aspects of healthcare delivery working within scope of practice under direction/supervision of a registered
