Full job description
Undertake duties relating to the care and personal care needs of the baby, child or young person. This will include intimate care, feeding play and general activities. Provide a high standard of personalise holistic home care to babies, children and young people with life limiting conditions. Ensure the safety of the baby, child or young person by monitoring and reporting all concerns and any changes to parents and the Integrated Nursing Service Lead as necessary.
Under the supervision of a senior member of staff provide a high standard of helathcare, always in partnership with the family. Adhere to personalised plan of care for each baby, child or young person and Haven House’s policies. Promote and sustain a suitable environment in which the baby, child or young person feels safe and comfortable, being aware of individual physical, social, emotional and spiritual needs. General To ensure that holistic care is family centred, research based and meets all statutory requirements.
To be responsible for exercising teamwork skills, working collaboratively with all members of the Haven House Care Team and other services supporting the family. To maintain an awareness of the multi-cultural nature of the Haven House catchment area, embracing diversity and promoting wider access for children, young people and their families. To act in accordance with Haven House policies, guidelines and standards. To keep up to date wit national policies and guidance To be responsible for further developing own knowledge and clinical skills and maintaining own competency.
To actively participate in reflective practice, mandatory training, appraisal management and practice supervision. To share information with colleagues at team meetings, when handing over care and at other informal contacts. To ensure that the views of children young people and families are brought to the attention of senior care staff for consideration and to inform service developments. To ensure that complaints are dealt with in a timely manner in accordance with hospice policy.
To maintain accurate records of all care provided. To act as advocate for the child when appropriate, maintain confidentiality. To Maintain clear and identified professional boundaries at all times To Report any equipment, maintenance or safety issues to the Facilities team and nurse in charge, ensuring that all Haven House property and equipment is well looked after and maintained. To carry out any other reasonable duty as requested by the Integrated Nursing Service Lead or Director of Clinical Services.
