Full job description
The team includes GPs, Pharmacists, Nurses, Advanced Nurse Practitioners and Administrators and the service is provided 8am – 8pm 7 days per week. The service is currently supported by local social enterprise provider London Central and West Unscheduled Care Collaborative out of hours (Mon-Fri, 6-8pm, Sat/Sun/Bank Holiday, 8am-8pm).
The service works closely with colleagues across ECP and Ealing’s Borough Based Partnership teams as well as Care Home providers with the aim of providing an integrated approach to the care available to the residents.
We are looking for someone with strong clinical assessment and decision-making skills with a compassionate approach to the older patient.
Provide clinical care to patients via face to face consultation sessions in various care home settings, telephone and video triage, telephone and video consultations), clinical administration, medical examinations and reports.
Provide accurate, contemporaneous and complete records of all patient consultations and clinical interactions, consistent with legislation, policies and procedures and in a format noting that entries will be provided to the care home and read by patients or their families.
Maintain open and effective communication with patients, their families, and caregivers, providing updates on treatment progress and addressing any concerns or questions.
Proactively contribute to and be able to chair multi-disciplinary team meetings including internal and external management meetings.
Promptly interpret, review and action diagnostic investigation such as ECG, X-Ray and Blood Tests
Work closely with colleagues across Ealing, including third sector organisations such as Marie Curie and Dementia Concern, and with other Ealing Community Partnership and Older Peoples services
