Full job description
Close liaison with patients, families, complex discharge team, social workers and therapists will be essential to ensure prompt but safe discharges of patients from our hospital setting
The job requires an enthusiastic individual with excellent communication skills to work in 7 South Gastroenterology Ward at Ealing Hospital. The successful candidate will need to facilitate communication between departments, interact confidently with patients and families, and ideally have a healthcare background.
The role involves planning safe patient discharges, improving bed utilisation, and reducing hospital stays. The successful candidate should become an expert in local homecare options, collaborate with various professionals, and have a clear understanding of safeguarding for in patient discharge processes. In conjunction with the Bed Manager, the post holder will also assist the multi-professional team giving support and advice and acting as a resource for all aspects of complex discharge planning.
