Full job description
The post holder will serve as a clinical expert and leader for the Inner London Diabetes Service, ensuring the delivery of high-quality, patient-centred care across Hammersmith and Fulham, Westminster (Central London), and Kensington and Chelsea (West London). They will be accountable for the clinical effectiveness of the service, bringing expert-level knowledge and skills to drive excellence in diabetes management.
The role demands a visible and accessible presence within the service, fostering strong relationships with patients, staff, and stakeholders while promoting best practices in clinical care.
As a pivotal figure in the service, the post holder will provide strategic clinical leadership, guidance, and direction to a multidisciplinary team, ensuring the development and implementation of innovative, evidence-based approaches to care. They will also act as a key resource for the diabetes service and the broader health community, championing improvements in nursing practice and health outcomes.
To work autonomously to deliver specialist clinical care appropriate to the needs of the patient group and based on principles that are evidence based and which enhance the quality of patient care.
To undertake comprehensive assessments of complex diabetes patients with co-morbidities, including physical examination, and to develop treatment plans for patients using investigative and analytical skills, and to formulate individualised management and treatment plans using clinical reasoning.
Review service achievements against relevant CLCH Quality Reports and organisational targets.
Contribute to the development and monitoring of clinical outcome metrics for related services including improved monitoring of patient experience.
- Line manages all team members (both clinical and administrative) in accordance with HR policies and procedures ensuring that good practice in recruitment, appraisal, performance management and other policies areas is maintained.
Monitor levels and standards of performance, staff conduct, standards of behaviour and staff absenteeism as appropriate ensuring the matters are dealt with effectively and in line with Trust policy and procedure.
Ensure that policies, procedures, clinical governance arrangements and lines of accountability are in place, appropriate and well communicated to all staff to ensure safe clinical service delivery.
