An opportunity has risen within Home treatment team (HTT) NW to support the team in the community. We have two full time roles .

Are you a compassionate and empathetic individual with a passion for supporting in the community in their own home environment? We would like to hear from you if you have good interpersonal skills, resilience and non- judgemental approach.

We provide holistic care, which includes physical health checks, identifying social stressors and sign posting patients to relevant services we closely work with.

You will part of a team which comprises of senior support workers, qualified nurses, Clinical Team Leads and a medical team.

The candidate must have a full and valid driving licence and access to a vehicle for work purposes due to the community element of this role.

1. To support the work of the team by ensuring that people experiencing a mental health crisis (and their carer) are offered prompt interventions that involve the least restrictive action and provide for individual physical, social and psychological needs.

2. To support the work of the team in undertaking visits and to provide intensive support and interventions to people using our services who are in crisis. To ensure that safe and effective interventions are offered and with discussion with senior colleagues in formulating care plans.

3. To participate in the operation of the hospital’s Assessment Suite to include providing a safe and respectful environment for the assessment of people detained on a Section 136 and people who are living in the community who may need inpatient admission.

4. To work collaboratively with the service user, carers, MDT and other relevant agencies to meet the identified need of the individual.

5. To work within the context of a multi-disciplinary team and ensure clear communication with people who use the service, their family or carers and colleagues.

To communicate with clients, their carers and other professionals, agencies and services to maximise care, promote crisis resolution and minimise risk
